• Identify and discuss the best practice models of primary and secondary prevention services in those chronic diseases where there is a significant remote variation – asthma, chronic obstructive pulmonary disease, diabetes, heart disease, stroke, amputation, and Alzheimers
• Identify the features of the services or systems that enable the model of care
• How the service environment and governance structures that support it work
• How the community is involved in the service model
• The key drivers and barriers for success.
